Circle of Care offers an innovative new concept in home care specifically designed for retirement communities: Shared Care Service Programs.

 

In today’s economy, retirement communities are seeking to provide creative new benefits and options to assist residents as they “age in place” in their independent living units. A Shared Care partnership with Circle of Care provides a companionship and personal care component to complement your Community Amenities.

 

Historically, senior community residents have hired the services of a private duty home care agency or individual to assist with activities of daily living and personal care to maintain their independence. The Retirement Community has little oversight in these situations and can sometimes face security concerns when these decisions are left to individual residents. Circle of Care staff and caregivers are carefully selected following reference checks, skills assessments, and a comprehensive criminal background check.

 

Most private duty agencies require that the individual contract for a minimum number of hours. This minimum can range from 2-5 hours per visit. Seniors who need assistance with bathing and dressing, or meal preparation, or medication reminders many times choose to go without assistance rather than pay for a number of hours of service that they do not feel they need. Your retirement community will find that Circle of Care’s unique Shared Care Service Programs will allow your residents to remain in place when needs for assistance with daily living arise.

 

Shared Care Service Programs are managed by Circle of Care including regular assessments by our nurse case manager. Shared Care Service Programs are designed to meet the 24-hour needs of residents and the specific market niche of the building or community. Circle of Care will work with each community to design a package of Shared Care Service Programs which provides one on one care to residents including Companion Care/Housekeeping Services and Personal Care Services that can be billed in hourly increments.  Alternatively, a Shared Care Service Program can be tailored to provide assistance to all residents in a designated wing or the whole community for a lump sum monthly fee.
